Initial Consultation

Your initial appointment will consist of an examination and the collection of diagnostic information. A consultation explaining your diagnosis and treatment options is often possible on the first visit. Occasionally, treatment can be done at the initial appointment, in cases of emergency. A dental prophylaxis (cleaning) is sometimes possible on the initial visit, as well. However, a complex medical history or treatment plan will require a more in-depth consultation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your initial appointment:

• Any X-rays if applicable

• A list of medications you are presently taking

IMPORTANT: A parent or guardian must accompany all patients under 18 years of age at the initial visit.

Please alert the office if you have a medical condition that may be of concern prior to your appointment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.
